Creative Minds CFS program services adults, 18-year-old and older, with a developmental disability who reside with family in the family home. CFS Case Manager will support with the coordination of caregivers, and will work alongside other Case Managers, Program Manager, and Program Director. CFS Case Managers will coordinate, and schedule client support and services, mentor and manage a team of direct care professionals, provide quality assurance, complete monthly, quarterly, and annual reports, and intake assessments.

Preferred candidate characteristics

• Knowledge/experience with the Regional Center and DDS protocol
• Field experience
• Fingerprint/background clearance required – no felonies or criminal record
• Acceptable DMV record
• Valid CA Driver’s License
• Working and reliable vehicle, auto insurance
• Health screenings clearance
• Eligible to work in U.S.

Pay and benefits

Rate of pay can be up to $24.00/hour based on experience and education. Workdays are Monday through Friday, 8 hours a day. Creative Minds offers an excellent benefit program which includes health insurance, dental, paid vacation, retirement account, life insurance, and more. Creative Minds offers participation in the DSP Stipend program through DDS which can earn you up to $1250 (before taxes). Also, a language stipend of $100 will be offered to bilingual staff.

Travel

Creative Minds provide services in the San Fernando Valley, Santa Clarita, and Antelope Valley areas. It is necessary to travel throughout these areas as part of the responsibilities associated with the position. A valid California Driver’s License, Car Insurance, and a reliable vehicle are requirements. As travel through the Valley is a necessary part of the position, a mileage allowance will be provided. Services will be provided in-person.
